The early ones were essentially just milk crates with roller skate wheels and perhaps a handle.
The wheels were the big issue, being often of clay, and ended in tons of accidents from slipping or breaking and causing a fall. Even with better boards the wheels still made difficulty, and the game pretty well died out in the mid-60's. Accidents and then the constantly increasing responsibility insurance finished the game a 2nd time and by the 1980's the majority of the parks had closed down. In the 1990's the game regained recognition and though it has had some fluctuations since it has especially grown and developed over the last few decades. Over time I have regularly heard it revealed this sport would not last. But it's developed from an activity into a relevant sport and now is widely followed around the planet. The targeting on tricks and jumps, great design on boards, clothing lines, shoes, and even music has reinforced the image. The S-Board is a board that twists in the middle and has only a single front and single rear wheel.
